1.( ) Enclosed is my check for 2008 CFMA market membership dues - $50 per market site. (You must join CFMA to obtain the group insurance, but are welcome to join CFMA without participating in the insurance program.)

2.( ) Enclosed is my check for CFMA group general liability insurance. The 2008 premium fee is $350 per market site. The 2008 sign up window closes March 29, 2007. All applications after that date must include a $25 insurance adjustment fee. You must also complete the insurance application form on the reverse side of this sheet. Please read the “CFMA Insurance Information Sheet” for 2008 changes to be sure you can comply with the insurance terms. Total fee (dues plus insurance) is $400 per market site before March 29, and $425 thereafter.

One check may be made out (payable to CFMA) for the total amount of membership dues and insurance premiums. Member dues and insurance premiums are not refundable.
